چکیده

This video demonstrates an endoscopic transsphenoidal resection of an corticotropin-secreting pituitary microadenoma. The patient is a 62-year-old man who was found to have a 5-mm pituitary adenoma after he exhibited signs and symptoms of Cushing disease. His endocrine laboratory studies confirmed central hypercortisolism. This video highlights dissection of the adenomapseudocapsule from the remainder of the pituitary gland for tumor removal. A gross total resection was achieved, and his postoperative cortisol levels were consistent with a biochemical cure of his Cushing disease. Figures and captions for relevant anatomy during tumor resection are depicted at the end of the video.

Endoscopic Endonasal Transsphenoidal Approach for Pituitary Adenomas: a Prospective Review of Our Early Experience

Background & Aim: Pituitary adenomas are a part of a dissimilar group of benign neoplasms. The development of endoscopic techniques for surgery of paranasal sinuses has increased the opportunity for an endoscopic approach with regard to the pituitary gland. Minimally invasive endoscopic pituitary surgery, in turn, permits a more thorough tumor resection and fewer associated surgical complicatio...

Three-dimensional endoscopic pituitary surgery.

OBJECTIVE We describe a novel 3-dimensional (3-D) stereoendoscope and discuss our early experience using it to provide improved depth perception during transsphenoidal pituitary surgery. METHODS Thirteen patients underwent endonasal endoscopic transsphenoidal surgery. A 6.5-, 4.9-, or 4.0-mm, 0- and 30-degree rigid 3-D stereoendoscope (Visionsense, Ltd., Petach Tikva, Israel) was used in all ...

Management of Pituitary Adenomas: Mononostril Endoscopic Transsphenoidal Surgery

Introduction: The endoscopic transsphenoidal approach for pituitary adenomas and other sellar lesions is quickly becoming the procedure of choice in their surgical management. The most common approach is binostril three-hand technique which requires a large exposure and subjects both nasal cavities to potential trauma. To reduce nasal morbidity, we employ a mononostril two-hand technique with t...
